Application Form for B.S. MLS

Apply to the Medical Laboratory Sciences Program

Application deadlines are June 15 for the fall admission and October 15 for the January admission.

Please make an appointment to contact a faculty representative of the program for an advisement to ensure that you have completed all the required prerequisites for the MLS program. This is required before application. Preference is given to those applicants who are legal New Mexico residents.

Applications are available online or can be mailed to you upon request.

Please make an appointment to contact a faculty representative of the program for an advisement to ensure that you have completed all the required prerequisites for the MLS program. This is required before application. Please download and save the file to your computer and complete. Once you have printed the completed form please mail it to Susan Dellinger at the address listed above.

Recommendation forms: Three recommendation forms must be filled out by appropriate persons. Please fill out the top portion and give the forms to appropriate persons along with a stamped envelope addressed to Susan Dellinger at the address listed above.
